New Patients

When booking your initial Pt evaluation please be prepared to provide the following information:

first/last name

phone number

email address (you will receive a link to set up your Jane profile as well as new patient paperwork)

date of birth

insurance information

please have your id and insurance card with you at the time of your first session. a copy will be scanned into your profile.

Existing Patients

please let the clinic know as soon as possible if there is a change in your insurance coverage or demographic information. Insurance claims may be denied if all information isn’t up to date.

payment

co-pays are due at the time of service

Insurance is a contract between each patient and their insurance company. MVP Movement & Performance PT is not a party in this contract. We will file claims on your behalf based on the information that you provide with us. You are responsible for knowing and understanding the requirements and limitations of your coverage. You as the patient are ultimately responsible for payment of all services provided by our clinic, which may include portions not covered by your health insurance plan.

cancellation

We ask that you cancel at least 24 hours in advance so we have a chance to schedule other patients who are in need of medical treatment.

same day Cancellations/no show no calls are subject to a 100% charge.
